Final Forms for Student-Athletes
For Student-Athletes in Grades 7–12
Copenhagen Central School District uses FinalForms, an online forms and data management system for student-athlete registration.
FinalForms allows parents/guardians to complete required athletic participation forms online. Information is saved from season to season and year to year, so families do not need to re-enter the same information each time.
Parents/guardians will be required to review and sign forms once per year and whenever information is updated.

Athletic Health Forms
To participate in interscholastic athletics, students must have current health forms on file with the Health Office.
According to New York State regulations:
A health examination for athletic participation is valid for 12 continuous months through the last day of the month in which the examination was completed. The physical remains valid for the duration of a sports season, even if the expiration date occurs before the season ends.

Become a Coach
Coaches play an essential role in the success of our athletic programs. They help student-athletes develop athletic skills while promoting leadership, sportsmanship, teamwork, responsibility, and character. Whether you are a certified teacher or a community member with a passion for working with students, we encourage you to explore coaching opportunities at Copenhagen Central School.
If you are interested in becoming a coach, please contact the Athletics Office for information regarding current openings and certification requirements.
New York State Coaching Certification
All interscholastic coaches in New York State must meet certification requirements established by the New York State Education Department (NYSED). Requirements vary depending on whether you are a certified teacher or a non-teacher coach, but all coaches are expected to complete required training to ensure the safety and success of student-athletes.

Coaching Education Pathways
New York State offers two approved pathways for coaching certification:
- NYSED Coaching Course Pathway – Complete the required coaching courses through an approved provider.
- NFHS Coaching Pathway – Complete the National Federation of State High School Associations (NFHS) coaching curriculum and internship.

Become a High School Sports Official

Section III is actively recruiting officials for a variety of sports and provides the training and resources needed to help you get started. Athletic Facilities
The athletic facilities at Copenhagen Central School are a source of pride for our school and community. Our athletic complex, including our synthetic turf field, is designed to provide a safe, high-quality environment for student-athletes, physical education classes, and community events.
To help preserve these facilities and ensure they remain in excellent condition for years to come, we ask that all athletes, coaches, parents, and visitors follow the guidelines below.
Synthetic Turf Field Guidelines
The following items and activities are prohibited on or around the synthetic turf field:
- ❌ Motorized vehicles of any kind, including ATVs, UTVs, golf carts (unless authorized), motorcycles, and dirt bikes.
- ❌ Electric recreational vehicles including electric scooters, hoverboards, and electric bicycles.
- ❌ Skateboards, roller skates, roller blades, and bicycles.
- ❌ Sunflower seeds, gum, peanuts, pistachios, and other food items that can damage or contaminate the turf.
- ❌ Tobacco products, vaping devices, e-cigarettes, nicotine products, alcohol, and illegal substances.
- ❌ Glass containers of any kind.
- ❌ Open flames, fireworks, grills, or smoking.
- ❌ Pets (except certified service animals).
- ❌ Metal cleats or other footwear that may damage the playing surface.
- ❌ Driving stakes, tents, umbrellas, or signs into the turf surface.
Facility Expectations
- ✔ Respect all school property and athletic equipment.
- ✔ Dispose of all trash in designated receptacles.
- ✔ Report any unsafe conditions or damage to school personnel immediately.
- ✔ Supervise young children at all times.
- ✔ Follow all directions provided by coaches, game management, and school staff.
- ✔ Remain in designated spectator areas during practices and contests.
- ✔ Help us keep our facilities clean, safe, and enjoyable for everyone.
