Welcome to Copenhagen Central School Athletics! 

Our athletics program is dedicated to developing student-athletes who demonstrate excellence both on and off the field. Through competition, teamwork, sportsmanship, leadership, and perseverance, we strive to provide meaningful experiences that help our students grow as athletes, students, and members of our community.

Final Forms for Student-Athletes

For Student-Athletes in Grades 7–12

Copenhagen Central School District uses FinalForms, an online forms and data management system for student-athlete registration.

FinalForms allows parents/guardians to complete required athletic participation forms online. Information is saved from season to season and year to year, so families do not need to re-enter the same information each time.

Parents/guardians will be required to review and sign forms once per year and whenever information is updated.

Athletic Health Forms

To participate in interscholastic athletics, students must have current health forms on file with the Health Office.

According to New York State regulations:

A health examination for athletic participation is valid for 12 continuous months through the last day of the month in which the examination was completed. The physical remains valid for the duration of a sports season, even if the expiration date occurs before the season ends.

Officials play an essential role in providing fair, safe, and competitive athletic experiences for our students.
